What are the responsibilities and job description for the START Coordinator position at Elevate CCBHC?
- Must be capable of regular and predictable attendance at a specific location to perform assigned tasks. Regular travel is required.
- Maintain an active caseload of individuals referred for services (variability occurs depending on the activity level of cases and the project scope).
- Ensure the coordination of support meetings and crisis plan development for individuals enrolled
- Provide outreach visits to the individuals’ home, day, and vocational settings regularly
- Use of clinical documents and other assessments for case planning and strategizing.
- Develop and maintain clinical documentation and any other applicable documentation of services provided.
- Develop materials and present community training as assigned by the director or designee, or when identified independently
- Complete all required data entry expectations, including adherence to timelines established by the program
- Provide on-call crisis response and assessment on a rotational basis
- Attend, participate and complete the Center for START Services Coordinator course, within the first year of employment
- Renewal of certification every two years after initial completion
