Classification: Part-time with potential for Full-Time

Job Type: Part-time to Full-Time

Pay: $19.00 - $26.00 per hour

Job Summary:

This is an entry, Bachelor degree, level position.

Interested in getting your SAC-IT, SAC? Elevate can provide supervision and hours! For individuals with a Master's and LPC-IT, Elevate can also provide supervision and hours for licensure!

Provide community based support for individuals working to establish and/or sustain a solid foundation for their recovery from mental health and/or substance use disorders. Assist clients in achieving independence and stability in their own residence. Depending on the particular client's needs, Case Managers visit the client at their residence on a regularly scheduled basis.

Respect and consideration given to the dignity of each client, visitor and fellow employee is a requisite of successful job performance. In addition, strict client confidentiality must be maintained.

Essential Functions:

- Developing, monitoring and supporting the client’s ability to maintain independence through the development and implementation of treatment goals/service plans, including obtaining necessary services.

- Provides direct client/staff interaction to develop and support improved lifestyle changes, including transportation of clients if necessary.

- Participate in staffing with both Washington County Human Services and Elevate

- Excellent communication skills including written documentation and verbal skills as well as time management skills.

- Empathetic and influential intervener in the patterns of client behavior.

- Promote a healthy balance between employment and leisure activities.

Qualifications:

1. A minimum of a Bachelor's degree in a related field is required

2. Experience in adult care, and mental health and alcohol and other drug abuse services, at least one year preferred

3. Experience and training as required under DHS 36 relative to psychosocial rehabilitation services is required

4. Knowledge of and experience providing services under DHS 36, Comprehensive Community Services for persons with Mental Health and Substance Use Disorders is preferred

5. Excellent communication and organizational skills

6. Strong interpersonal skills

7. Knowledge of mental health diagnoses and treatment, CBT, and/or psychopharmacology is preferred

8. Knowledge of patient rights and privacy laws

9. Valid Wisconsin Driver's License and eligibility for employer's automobile insurance coverage is required.
