Today, Elica is a network of nine health center facilities located in Sacramento and West Sacramento.  Approximately 10% of Elica's patients receive care outside our clinic facilities through the Wellness Outside Walls (WOW) initiative, which includes our Street Medicine and Mobile Medicine programs.

Elica proudly stands as a Federally Qualified Health Center (FQHC), a designation given by the Federal government to community-based clinics that meet a rigorous set of requirements. Our mission is to provide high quality, affordable health care through the operation of professional and compassionate health homes for under-served populations, who are confronted with barriers to accessing care.

The Family Nurse Practitioner (FNP) will deliver family-focused care by providing comprehensive medical services to patients in association with other members of Elica's health care team.  We are currently seeking a family nurse practitioner to provide care to our homeless population.  


Requirements

Job Duties and Responsibilities


? Direct and indirect patient care including, but not limited to, observation of signs and symptoms of illness, reactions to treatment, general behavior, or general physical condition, and determining of whether the signs, symptoms, reaction, behaviors, or general appearance exhibits abnormal characteristic; and implementation, based on observed abnormalities, of appropriate reporting, or referral, or standardized procedure, or changes in treatment regiment in accordance with standardized procedures, or the initiation of emergency procedures.

? Direct and redirect patient care services that ensure the safety, comfort, personal hygiene, and protection of patients; and the performance of disease prevention and restorative measures.

? The performance of skin tests, immunization techniques, and the withdrawal of blood from veins and arteries.

? Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients.

? Performs general physical examination as indicated and instructs patients in health promotion and disease prevention.

? Examines patients to determine the presence of illness, utilizing physical findings, laboratory results, medical history, ascertains nature and extent of illness and prescribes treatments as indicated.

? Documents and maintains patient records of services provided according to agency and State of California standards, and completes records on a daily basis.

? Completes and submits billing in an agreed-upon and timely manner.

? Develops and institutes appropriate treatment plans per protocol with the consultation of Medical Director, and/or supervising physician as needed. Treatment will include the care of acute and chronic medical problems.

? Provides and assists with various counseling and education functions.

? Participates in the development of protocols.

? Establishes a collaborative relationship with other medical providers.

? Obtains appropriate consults when problems presented are outside the scope of practice or area of expertise, or when unfamiliar with a particular aspect of the patient’s care.

? Answers messages and sign off on clinical reports (consults, labs, x-rays, etc.) in a timely manner.

? Participates and conducts in continuing education courses, in-services, and staff meetings. Attends other meetings as assigned.

? Knowledge of resources to assist homeless persons in the greater Sacramento area, and participation in community homeless service meetings as requested.

? Participates in Performance Improvement, Quality assurance programs and Peer review.

? Productivity satisfactory and consistent with the clinical goals.

? Provides cultural and linguistically appropriate services to a diverse and socioeconomically challenged population.

? Responsible for other related job duties as assigned by the Medical Director.

Requirements


? Graduate of an accredited school of nursing - Masters degree (Required)

? Current California State NP licensed ( Required) 

? Current Nurse Practitioner Furnishing Number (Required)

? Active DEA registration (Required)

? Certification with a nationally recognized certification agency such as AANP or ANCC 

? Current Basic Life Support - Health care professional level

? At least 1-2 years of clinical experience 

? Homeless population experience preferred


Work Environment 


The work environment is primarily field-based triaging and administering medical care to patients experiencing homelessness. Work environment includes streets and surrounding areas of Sacramento County; homeless shelters; and health clinics. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to walk; use hands to finger, handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment can range from quiet to moderately loud; the incumbent must be able to focus in an environment with many distractions.

The employee may be in contact with individuals and families in crisis who may be ill, using controlled substances and/or not attentive to personal health. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bio-hazardous materials (bodily fluids including blood and urine) and hazardous chemicals. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations.

Mobile Medicine and Street Medicine require comfort providing medical care outside of the usual clinic setting.  Resources may be limited compared to a fixed-site office clinic, particularly regarding support staff, diagnostic testing, supplies and equipment, and space.  There may be increased exposure to outside elements, including sun, heat, cold, and rain.  Employees will be expected to dress and prepare accordingly.
