General Description:
Performs basic care procedures. Recognize the elderly we serve as unique persons, requiring individual alternatives to maintain full lives in their later years. Provide medical, spiritual, and emotional care; encompassing quality, dignity, compassion and confidentiality for each individual.
Essential Job Functions:
- Communicates with resident, families and staff in a sensitive, professional and therapeutic manner.
- Observes and communicates changes in the patient condition to the RN responsible for the care of the assigned resident.
- Takes resident’s vital signs and records on the appropriate form.
- Assists in the resident activities of daily living which includes assisting with meals and nourishments, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing and dressing.
- Documents care and observation on appropriate forms.
- Assists with admission, transfer and discharge of resident.
- Provide for the safety and security needs of assigned residents.
- Maintains established departmental policies, procedures and objectives, quality assessment and improvement programs, and safety, environmental and infection control standards.
- Attends meetings as required.
- Participates in education programs and inservice meetings.
- Understands and follows “Universal Precautions” as it pertains to potential exposure to all body fluids. Utilizes infection control techniques.
- Understands chemicals(s) used in their department under the “Right to Know” standard.
- Reports all unsafe conditions to appropriate supervision, including change in patient status, broken or malfunctioning equipment or anything that could be a potential threat, danger to a patient. Practices good safety awareness.
- Knowledgeable of Residents Rights and facility Abuse Policies and Procedures.
- Performs any other duties and responsibilities that may become necessary or as directed by Supervisor.

Qualifications

Entry Level Qualifications:
- Post High School Vocational/Specialized Training
- Current State of Connecticut CNA certificate; CPR certification preferred.
- PAC Training essential.
Physical Requirements:
- Good physical and mental health.
- Extremely heavy physical effort (lift/carry up to 50 lbs.)
- Prolonged, extensive, or considerable standing/walking
- Lifts, positions, pushes, transfers residents.
- Manual dexterity and mobility
- Intermittent exertion when performing treatments
- Considerable reaching, stooping, bending, kneeling, crouching
- Ability to adjust vision to bring objects to focus

Key Accountabilities (with or without reasonable accommodation):
- Demonstrates the ability to provide personal hygiene for residents by bathing, oral hygiene, denture care, hair and nail care, skin care, incontinent care, dressing and undressing.
- Demonstrates ability to provide nourishment by serving trays, providing assistance in the resident dining room, feeding residents, measuring and recording intakes, serving nourishment, use of Thick-It and anti-choking procedures.
- Demonstrates ability to provide incontinent care to residents by application and disposal of diapers, use of bed pans and urinals, toileting resident with or without assistance, measuring and recording of output, specimen collections and peri care
- Demonstrates ability to take temperature, pulse, blood pressures, and use of scales for weighing and mechanical lift.
- Ability to recognize resident change in condition and notify charge nurse.
