The Behavioral Health Division at Eliot provides a comprehensive network of evidence-based services to adults with psychiatric disabilities, offering rehabilitation & hope to those in need.
PACT staff coordinate and deliver recovery and rehabilitative services to consumers participating in PACT, addressing therapeutic & behavioral interventions for individuals with severe mental illnesses & substance abuse disorders.
Services are primarily community-based, taking place at consumers' homes, medical facilities, and other locations.
Responsibilities:
- Provide case management services as part of a multidisciplinary team
- Act as the primary point of contact to mitigate housing issues as they arise
- Develop treatment plans with consumers
- Serve as the primary case manager to consumers in accordance with Eliot's policies and PACT standards
- Participate in completing assessments assigned by the Team Leader or Lead Clinician in a timely manner
- Assess clients' functions, strengths, and needs
- Establish therapeutic connections with each client and provide clinical interventions
- Offer problem-solving supports and interventions to clients
- Provide on-call support on a rotating basis and participate in evening shifts one day per week, along with one weekend shift per month
Schedule: Monday - Friday 8am to 4pm, with one 12pm-8pm shift per week and one weekend shift per month. Rotating on-call duties and a rotating holiday schedule apply.
