Eliot is seeking a Nurse Care Manager to provide medical care coordination as part of its multidisciplinary BHCP team. The Nurse CM will provide and ensure medication reconciliation and engage with medical providers of all enrollees. Assist enrollees in engaging with medical providers, recommendations and other types of health / medical supports. Provide education and consultation to enrollees and CP team members regarding medical, health and medication related topics. Perform medical tasks as needed and directed interventions to enrollees related to their treatment plan. Provide services and supports for up to 250 enrollees. Additionally, candidate will possess strong time-management, organizational and communication skills allowing for some work related tasks to be completed remotely
Eliot's Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation and hope to those in services. Eliot's Behavioral Health Community Partner (BHCP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.
Responsibilities :
Schedule : Mon-Fri, 9am-5pm
Location : Primarily hybrid. Travel required for face-to-face meetings. Mileage reimbursed
Salary : $70,000.00
