PACT Team Member Responsibilities
- Deliver case management services as part of a collaborative team effort.
- Take a proactive approach in mitigating housing concerns, serving as the central contact for consumers.
- Work closely with consumers to develop personalized treatment plans, tailored to their specific needs.
- Function as primary case managers, ensuring compliance with Eliot's policies and PACT standards.
- Participate in assessment completion, as assigned by the Team Leader or Lead Clinician, in a timely and efficient manner.
- Evaluate client capabilities, strengths, and needs to provide informed support.
- Nurture therapeutic relationships with clients, offering clinical interventions to promote recovery.
- Supply problem-solving assistance and interventions to help clients overcome obstacles.
PACT Schedule and Expectations
The PACT program operates from Monday to Friday, 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Rotating on-call duties and holiday schedules are also expected.
