What are the responsibilities and job description for the Recovery and Rehabilitation Case Manager position at Eliot's?
Eliot's Behavioral Health Division offers a range of services aimed at supporting adults with severe mental illnesses and/or substance abuse disorders. Our PACT staff plays a pivotal role in delivering comprehensive recovery and rehabilitative services.
PACT Staff Key Responsibilities
- Deliver case management services as part of a cohesive team.
- Proactively address housing concerns, serving as the key liaison for consumers.
- Collaborate with consumers to develop tailored treatment plans, addressing their unique needs.
- Function as primary case managers, upholding Eliot's policies and PACT standards.
- Contribute to assessment completion in a timely fashion, as designated by the Team Leader or Lead Clinician.
- Evaluate client capabilities, strengths, and needs to supply targeted support.
- Foster therapeutic relationships with clients, offering clinical interventions to facilitate recovery.
- Provide problem-solving assistance and interventions to assist clients in overcoming obstacles.
PACT Program Overview
The program operates Monday through Friday, from 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Rotating on-call responsibilities and holiday schedules are also expected.
