Eliot's Family Networks Service is currently seeking a Service Coordinator to assess and review referrals from the Department of Children and Families. The Service Coordinator is also responsible for overseeing the step-down of clients from residential to home placement. The ideal candidate will have three years of clinical / child welfare experience, including experience in permanency planning in a child welfare setting and knowledge of system of care practices.

Our Social Services Division includes short-term community-based assessment programs and group homes for children (birth to 12 years old) and adolescents (13 to 21 years old). The programs are contracted with the Department of Children and Families and provide a safe, structured, and respectful environment for children and youth to thrive, change, and grow by focusing on treatment, skill building, and permanency.
Responsibilities :
- Assess and review DCF referrals to be processed appropriately.
- Utilize designated Level of Service Tool to evaluate appropriate referrals to step-down clients from residential to home placement.
- Oversee emergency placement to shelter / specialized care (respite pre-residential).
- Oversee step-down of youth from residential to home placement.
- Organize and lead Family Networks diversion teams.
- Make appropriate network and community service referrals.
- Coordinate services with specialized service managers.
- Facilitate Continuous Quality Improvement / outcome tools.
- Link DCF service plans and network provider treatment plans including coordination with foster care reviews, DCF clinical conferences, etc.
- Write up and record treatment plans for Family Networks families.
- Respond to crisis management needs of the area DCF office.
Schedule : Full-Time, Monday-Friday 9am-5pm
Pay : $48,000 annually
