Life Care Manager

Life Care Advocates, formerly know as Elite Life Care Services, provides complete guardianship and life care services. We are hiring a LIFE CARE MANAGER due to company growth in the Naples area! This role meets with our clients and wards in the field and requires a home office to work from.

Responsibilities may include and are not limited to:

· Assess, evaluate, and create a personalized Life Care Plan that promotes each client to reach their optimal level of success in their quest to attain the highest quality of life according to their own distinct individual needs.

· Monitor the CARE plan from the inception, guiding the client to their personalized journey through life.

· Serve as the "eyes and ears" for the geographically challenged family.

· Evaluate living options for the client.

· Advocate for the optimal care for the client in all settings, including hospitals, assisted living and long term care communities.

· Provide crisis and restorative management for the challenging events that consume the family.

· Provide mediation and restorative guidance to those clients with challenging family dynamics.

· Coordinate reporting to Attorneys, Fiduciaries, Health Care professionals, and Family members.

· Coordinate physician and dental appointments for the client.

· Coordinate major events for the client such as vacations, and family events .

· Advocate and file for government entitlements, Medicare, Medicaid, and Social Security Disability

· Provide wellness checks for the client.

· Manage the daily needs such as bill paying, housekeeping, social outings, and groceries.

Experience/Requirements:

· MSW preferred or guardianship experience and a minimum of 2 years of related experience

· Excellent verbal and written communication skills

· Proven ability to work independently as well as in a team environment

Pay: $60,000.00 - $70,000.00 per year
