Where You’ll Work:
While our office is located in Warsaw, Indiana, this position is based out of our Logansport office and serves clients across the state. Regular in-person presence in Warsaw will be required several times a week for team collaboration and oversight. A company vehicle is available for travel and in-home visits.
What You'll Do:
- Support Clinical Leadership – Serve as the Alternate to the Director of Nursing during absences or state surveys.
- Provide Skilled Nursing Care – Conduct assessments, oversee care plans, and educate families and caregivers.
- Drive Quality and Compliance – Ensure adherence to Medicaid, PA, and state guidelines.
- Mentor Nursing Staff – Train and support clinical team members to deliver consistent, high-quality care.
- Represent the Brand – Build trust with patients, families, and community providers.
What We're Looking For:
- Active RN license in Indiana (Required)
- 3 years of clinical experience (home health preferred)
- Prior leadership experience is a plus, but not required
- Knowledge of home health regulations, Medicaid, and PA processes (preferred)
- Strong communicator, self-starter, and team player
- Comfortable with light travel during the week (vehicle provided)
