We are seeking a Clinical Nurse Specialist - Home Health to join our team at Elite Home Health. As a CNS, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans, promote healthy behaviors, and improve patient outcomes.
About You
We are looking for a nurse leader who is passionate about delivering high-quality patient care, has excellent communication and interpersonal skills, and can work effectively in a fast-paced home health environment. The ideal candidate will have a Master's degree in Nursing, a current RN licensure in the state of practice, and current CPR certification.
Key Responsibilities
- Develop and implement care plans in collaboration with patients, families, and healthcare professionals
- Promote healthy behaviors and educate patients and families on self-care techniques
- Collaborate with interdisciplinary teams to improve patient outcomes and quality of care

Compensation and Benefits
The estimated salary for this role is $80,000 - $110,000 per year, depending on experience. In addition to competitive pay, we offer a range of benefits, including health insurance, retirement savings plan, and paid time off.
