The Home Health Nurse Specialist provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Key responsibilities include coordinating total patient care with other healthcare professionals, achieving continuity through effective planning and communication, and ensuring accurate documentation in collaboration with physician orders.
- Makes initial and comprehensive nursing evaluations to determine primary care focus, develops the plan within state-specific guidelines, and submits timely documentation.
- Ensures patients meet home health eligibility and medical necessity guidelines defined by payer sources.
