About the Role:
- Patient Care Manager Responsibilities:
The Patient Care Manager will oversee the supervision and coordination of clinical services, ensuring that patients receive high-quality care in their homes as prescribed by physicians. This involves providing and directing nursing care in accordance with applicable laws, regulations, and agency policies.
- Key Responsibilities:
* Provides clinical services within the scope of practice, defined by state laws governing nursing, in coordination with the plan of care and other healthcare team members.
* Receives referrals, assigns clinicians, evaluates patient orders, and plots start-of-care visits.
* Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
* Ensures patient needs are continually assessed, and care rendered is individualized to meet patient needs, reasonable, and in accordance with physician orders.
* Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as necessary.
Requirements:
- Education & Experience:
* Current RN licensure in the state of practice
* Current CPR certification required
* Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
