Position Summary
Elite Nursing & Homecare is seeking an Alternate Clinical Manager, RN (Field Nurse) to support our clinical leadership team and deliver high-quality skilled nursing care in patients’ homes. This role serves as the primary alternate to the Director of Nursing (DON) and Alternate DON when they are unavailable for state surveys, regulatory audits, or other key responsibilities.
As an Alternate Clinical Manager, RN, you will play a pivotal role in ensuring compliance with state regulations, managing clinical operations, and providing direct patient care in the field. This is an excellent opportunity for an experienced and motivated RN who is passionate about both leadership and hands-on patient care.
Key ResponsibilitiesLeadership & Compliance:
- Serve as the designated alternate to the Director of Nursing (DON) and Alternate DON during state surveys and regulatory reviews.
- Support clinical and administrative operations to ensure compliance with state regulations, Medicaid requirements, and Prior Authorization (PA) processes.
- Oversee quality assurance programs, participate in staff training, and ensure adherence to agency policies.
- Act as a resource and mentor for the nursing team, fostering a collaborative, professional, and supportive work environment.
Field-Based Patient Care:
- Conduct in-home patient assessments and provide skilled nursing care, including treatments, medication management, and patient education.
- Develop, implement, and evaluate individualized care plans tailored to each patient's needs.
- Maintain accurate medical records in accordance with company policies and regulatory requirements.
- Build and maintain strong relationships with patients, families, and community healthcare providers.
Qualifications:
- Active Registered Nurse (RN) license in good standing in the state of Indiana.
- Minimum of 3 years of nursing experience, preferably in home health, skilled nursing, or a similar setting.
- Prior supervisory or administrative experience is preferred.
- Strong knowledge of home health regulations, Medicaid guidelines, and Prior Authorization (PA) processes.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work independently while also being an integral part of the clinical leadership team.
- Valid driver’s license and reliable transportation for in-home visits.

Schedule & Work Environment:
- Full-time position
- Monday – Friday, day shifts (with occasional weekend and on-call responsibilities as needed)
- Work Location: In-person in Warsaw, IN (field-based role)
Job Type: Full-time
Pay: $77,000.00 - $86,000.00 per year
