We are seeking an experienced Registered Nurse to join our team at Elite Hospice. As a Hospice RN, you will be responsible for providing comprehensive care to patients and their families, focusing on symptom management, pain control, and emotional support.
The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. A minimum of one year of clinical experience is required, with current licensure in the state of practice and CPR certification.
In this role, you will have the opportunity to develop your skills as a case manager, coordinating patient care and collaborating with the interdisciplinary team to ensure seamless delivery of services.
- Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care daily
- Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
