Job Description
We are hiring a skilled Registered Nurse (RN) to join our team as a Hospice RN Case Manager. As an end-of-life care expert, you will develop trusting relationships with patients and their families, providing compassionate and personalized care.
In this role, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, evaluating personal and professional performance continuously to increase productivity and quality of care delivered.
You will make initial nursing evaluations to determine eligibility for hospice services, identifying physical, psychosocial, and environmental needs of patients and families. Your duties will include:
- Conducting regular assessments to identify changing needs and re-evaluate the plan of care as necessary
- Initiating and coordinating the plan of care, maintaining accurate and relevant clinical notes regarding patient condition
- Collaborating with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) in providing patient and family care
To be successful in this role, you must have one year of clinical experience, current RN licensure in the state of practice, and CPR certification. You should also possess a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
