Key Responsibilities:
In this role, you will:
- Provide full nursing responsibility for delivering high-quality care to each hospice patient, continuously evaluating performance and making necessary adjustments to increase productivity and quality of care.
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs and reassess regularly, no less than every 15 days.
- Initiate and coordinate individualized plans of care while maintaining accurate clinical notes regarding each patient's condition.
- Document problems, goals, interventions, and patient/family response to hospice care.
- Collaborate daily with patients, families, attending physicians, and other members of the interdisciplinary group to provide seamless patient and family care.
Experience Requirements:
We seek candidates with:
- At least one year of clinical experience in a similar setting.
License Requirements:
To be successful in this role, you must have:
- Current RN licensure in the state of practice and at least one year of clinical experience.
- Current CPR Certification.
- A valid driver's license, vehicle insurance, and access to reliable transportation.
Salary Range:
$85,000 - $110,000 per year.
