Key Responsibilities:

In this role, you will:

1. Provide full nursing responsibility for delivering high-quality care to each hospice patient, continuously evaluating performance and making necessary adjustments to increase productivity and quality of care.
2. Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
3. Identify patients' physical, psychosocial, and environmental needs and reassess regularly, no less than every 15 days.
4. Initiate and coordinate individualized plans of care while maintaining accurate clinical notes regarding each patient's condition.
5. Document problems, goals, interventions, and patient/family response to hospice care.
6. Collaborate daily with patients, families, attending physicians, and other members of the interdisciplinary group to provide seamless patient and family care.

Experience Requirements:

We seek candidates with:

• At least one year of clinical experience in a similar setting.

License Requirements:

To be successful in this role, you must have:

• Current RN licensure in the state of practice and at least one year of clinical experience.
• Current CPR Certification.
• A valid driver's license, vehicle insurance, and access to reliable transportation.

Salary Range:

$85,000 - $110,000 per year.
