What are the responsibilities and job description for the RN Hospice Case Manager position at Elite Hospice?
Responsibilities:
- Provide direct patient care in the Patient's Home, Personal care Homes, and Nursing homes
-Educate patients and families regarding Hospice, Plan of Care, and What to Expect at end of life
- Administer/Order medications and treatments as prescribed by physicians
- Utilize the electronic medical record system to document patient care
-Perform Wound Care as needed
- Practice aseptic technique to prevent infection
- Provide home care services to patients as required
- Implement infection control measures to maintain a safe environment for patients
Requirements:
-Reliable transportation
- Must be a Registered Nurse with a heart to serve patients
- Proficient in medication administration and education
- Knowledge of aseptic technique and infection control protocols
- Experience in Hospice is desirable but not required
- Strong communication and interpersonal skills
- Ability to work effectively within a team and independently
