Client Care Specialist (Caregiver)

MAIN FUNCTIONS: To perform personal care tasks and household work related to the care of the aged or infirmed individual (client)

ESSENTIAL TASKS:

• Assist Client with the following Personal Care Routines:
• Mouth care, denture care
• Grooming (shampoo, dry and comb hair, shave, ordinary care of nails)
• Bathing (bed bath, tub bath, shower)
• Elimination (bedpan, commode, toilet)
• Transferring (bed to chair, bed to wheelchair)
• Meals-prepare, serve, assist with feeding as needed) a well-balanced diet
• Dressing and undressing activities
• Self-administered medications (remind client to take medications)
• Ambulation and movement (assist to avoid falls, safety)
• Perform household tasks essential to the client's needs as they relate directly to the client's comfort and well-bein:
• Light housekeeping
• Make beds, change linens
• Dust and vacuum client's room
• Tidy kitchen, bathroom
• Wash dishes after meals
• List needed cleaning supplies for the home (paper goods, also)
• Wash client's laundry
• Shopping, opening mail, banking and errands, if no other management is possible.
• Perform other health related tasks which may be delegated by a physician, an R.N., or other health care professionals,
• Advise the Agency staff of any incidents in the home and/or changes in the client's status.
• Other reasonable, acceptable tasks which may be requested by a client who is self-directing, or the client's family. These tasks may include observation of the client, recording and reporting facts about the client's behavior, and carrying out appropriate procedures and techniques, as appropriate.
• Ensures that the rights of the client are maintained.
• Reports to work and works as scheduled and assigned by the client and field supervisor.
• Reports to the Coordinator at the office in advance of changes to the schedule or absence from work.
• Attends staff conferences and in-service meetings as scheduled.
• Represents the Agency in a loyal manner and dresses in appropriate clothing.
• Completes service delivery record (time sheet) on the day of each visit and turns in semi-monthly time sheet as soon as possible after the last day worked during the pay period (NOT APPLICABLE TO PROVIDERS PARTICIPATING IN ELECTRONIC VISIT VERIFICATION (EVV).
• consistently and accurately record time worked for each client’s scheduled hours.
• Other duties as assigned.

Physical Qualifications:

• Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.
• Requires prolonged standing or sitting
• Requires frequent bending, stooping or stretch
• Requires corrected vision and hearing within functional levels and the absence of color blindness.
• Able to verbally communicate effectively.
• Requires extensive daily documentation.
• Able to work in stressful environments.
• Able to work the hours required for duties and responsibilities.

QUALIFICATIONS REQUIRED:

• Must be 18 years of age.
• Must be able to demonstrate competency to perform caregiver tasks above.

SKILLS AND ABILITIES:

• Demonstrated ability to read, write and communicate effectively.
• Ability to complete service delivery record (time sheet).
• Ability to follow specific directions related to personal care tasks and light housekeeping duties.
• Availability of personal, reliable transportation.
• Able to work in various and stressful environments.
• Able to relate well with clients and families.
• Able to work as scheduled and report to work precisely at the scheduled time.
• Able to lift a minimum of 25 pounds.
• Able to push and pull up to 50 pounds.
• Able to analyze and handle emergency situations.
• Able to manage time wisely and complete tasks daily.
• Able to understand and follow universal precautions.
• Able to read, understand and comply with all company policies and procedures.
