Position Summary
The Hospital Liaison plays a key role in coordinating patient transitions from hospitals to our skilled nursing facility. You will serve as the face of our organization to hospital discharge planners, case managers, patients, and families—helping them navigate the post-acute care process with empathy and efficiency.
Key Responsibilities
Promote our facility’s services and maintain census goals.
Develop and maintain strong relationships with hospital discharge planners, case managers, and physicians.
Participate in marketing events, community outreach, and referral development.
Evaluate potential residents in hospital settings and assess clinical needs for appropriate placement.
Coordinate admission processes and ensure a smooth, timely transition to the nursing home.
Communicate effectively with families, hospital staff, and internal care teams.
Qualifications
2 years of experience in a Hospital Liaison or Marketer role for a SNF
Strong interpersonal and communication skills
Knowledge of Medicare/Medicaid and insurance requirements for skilled nursing admissions
Reliable transportation and willingness to travel to local hospitals
