We are seeking a dedicated Social Worker 2 to join a Clinical Care Team focused on addressing social determinants of health for primary care patients. This role involves collaborating with healthcare providers to ensure patients receive essential social support and resources, conducting case management activities, and facilitating access to medical services and low-cost prescriptions.
Key Responsibilities :
- Navigate social support resources for food, housing, and mental health services.
- Conduct assessments and case management for patients, including discharge planning from hospitals.
- Assist with specialty referrals, including scheduling and tracking.
- Help patients access low-cost prescription options and support program applications.
- Document patient interactions and engage with community health initiatives.
