Responsibilities of a Social Worker :

Conduct psychosocial assessments of residents to identify their needs and develop appropriate care plans.

Provide counseling and emotional support to residents and their families.

Coordinate and facilitate support groups and educational programs for residents and families.

Collaborate with healthcare professionals to ensure holistic care for residents.

Advocate for residents' rights and ensure their needs are being met.

Assist residents in transitioning to different levels of care or discharge planning.

Maintain accurate and up-to-date documentation of resident interactions and care plans.

Participate in interdisciplinary team meetings to discuss resident care and progress.

Qualifications of a Social Worker :

Bachelor's or Master's degree in Social Work (BSW / MSW) from an accredited program.

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Pennsylvania.

Experience working in a healthcare or nursing home setting is preferred.
