Elliot Hospital Inpatient Pharmacy
The inpatient pharmacy department is comprised of pharmacists and technicians working collaboratively to provide safe, timely and effective medication therapy to our patients. We operate a decentralized model for both medication distribution utilizing Omnicell automation, as well as clinically with pharmacists dedicated to the teams on our nursing units. About The Job
The Pharmacy Technician III – Medication Reconciliation is a technician with a specialized role responsible for conducting operational and technical elements of obtaining and transcribing medication history information related to the medication reconciliation process.
What You'll Do
High School Diploma or equivalent required, advanced degree preferred. Certified as a Pharmacy Technician required. ASHP Medication History Taking and Reconciliation Certification (or approved medication reconciliation certification) required within 6 months of hire.
A minimum of one year pharmacy experience required. Experience with Medication Reconciliation preferred. Experience in hospital pharmacy preferred.

The Pharmacy Technician III – Medication Reconciliation is a technician with a specialized role responsible for conducting operational and technical elements of obtaining and transcribing medication history information related to the medication reconciliation process.
What You'll Do
- Gather and document most up-to-date patient home medication list through multiple sources including, but not limited to, patient interviews and calls to caregivers, pharmacies, and provider offices.
- Effectively communicate with patients, caregivers, and healthcare providers to assess significant medication list changes, allergies, and any other pertinent information to relay to pharmacist and healthcare team.
- Discern key information that should be passed to the clinical care team for interpretation and assessment.
- Accurately and professionally document medication list and associated notes in the electronic health record.
- Perform duties of Pharmacy Technician I and/or Pharmacy Technician II as necessary, based on staffing needs.
- Review the Prescription Drug Monitoring Program (PDMP).
- Identify discontinuation of medications.
- Perform other duties as assigned.
