Job Summary:
We are seeking compassionate and dedicated Caregivers, Support Employment and Living Coaches to join our team. As a caregiver, you will provide basic care and support to individuals with special needs and disabilities in their homes. Your role will involve in assisting with daily activities, light housekeeping for the consumer only, and ensuring the overall well-being of our clients. We are contracted with APD which is the agency for persons with disabilities.
Responsibilities:
- Provide personal care assistance to consumers, including bathing, grooming, and dressing
- Assist with mobility and transfers
- Monitor and record vital signs, such as blood pressure and temperature if needed
- Supervise medication being taken by the consumer as directed by healthcare professionals
- Assist with meal preparation and feeding
- Perform light housekeeping tasks to maintain a clean and safe environment
- Provide companionship and emotional support to consumer
- Collaborate with agency to develop and implement care plans
- Assist with exercises and therapy programs as prescribed
- Maintain accurate documentation of consumer care activities
-Assist individuals with living arrangement goals
-Assist individuals with employment goals such as preparing for a job interview, getting through the hiring process, maintaining employment, and transistion from one employer to another
-Assist with life skills and development
-Assist with social skills and social environments
Experience:
- Previous experience in a similar role preferred
- Familiarity with medical terminology and procedures
- Knowledge of working with individuals with developmental disabilities is a plus
- Experience in nursing homes or hospice care settings is beneficial

Qualifications:
-Competent with smart phones to clock in & out in compliance with medicaid requirements (experience with Iconnect is a plus)
-Local background check from a police department or sheriff's office
-Level 2 background screening
-High School diploma, C.N.A license or HHA certificate
-Auto registration & proof of auto insurance
-Professional liability insurance
-CPR certification
-Physical examination within the last 6 months to show free and clear of communicable diseases.
-Must be willing to drive to outside of Broward to Boca, Delray, Palm Beach the furthest.
-Experience with feeding tubes, be able to lift or transfer the client from bed to chair or chair to bed.
-Experience with Autism, down syndrome and other intellectual disabilities.
-Trained and certified as a Life skills coach
