Elmwood Assisted Living of Fremont is now hiring for an Assistant Health Services Coordinator (LPN) for our Full-Time position (30-35 hours a week). Full benefits package! Active Unencumbered LPN license required. This position does require on-call rotation and will include some off shifts like 2nd and 3rd shifts and will include covering the floor as necessary.

Job Summary:
The Assistant Health Services Coordinator (AHSC) is directly responsible for the recruitment, hiring, training, scheduling, and monitoring of care managers. The AHSC is responsible for identifying the need for and hiring care managers. The AHSC is responsible scheduling the care managers. Directly responsible for the initial and on-going training for care managers. This position is responsible for monitoring the quality of services provided by the care managers. Written plans for each resident will be developed and monitored by the AHSC. Understands and follows all Elmwood policies and procedures. Completes forms with accurate documentation for payroll entry as assigned. Responsible for doing admissions and discharges as necessary for residents. Responsible for maintaining an organized workflow for charts and resident and staff documentation. Completes any other duties as assigned by the Executive Director.
Responsibility For:
Nursing and personal care of the Assisted Living residents and ensuring that good quality care is provided to all residents. Looks after and provides for care and welfare of the residents in a clean, cheerful and healthful environment. Orders supplies and mail order medications that are needed for each resident. Writes and oversees all resident-focused plans. Completes wellness visits. Recruits and hires all care managers as needed or assists the DAS with recruitment and hiring as per facility rule. Provides orientation training for all care managers, and assures that on-going training is provided as needed. Supervises, disciplines, and completes performance reviews for the Care Managers as approved by the Executive Director to help insure good quality of care to the residents. Responsible for off hours and weekend staff coverage in conjunction with the other department leaders as assigned on a rotating schedule per facility rule. Flexes hours to spend time on various shifts. On call as necessary for the smooth flowing of staffing for the Assisted Living.
Requirements:
LPN license from an approved nursing school.
Working Conditions:
Works in well-lighted and ventilated office area as well as throughout the nursing facility. Sits, stands, lifts and moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government personnel, etc., under all conditions and circumstances. Is subject to hostile and emotionally upsetting situations at times. Must be willing to work beyond normal working hours and on weekends. Is subject to falls, burns from equipment, odors, etc., throughout the workday. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the AIDS, Hepatitis B and CODIV-19 viruses.
Specific Requirements:
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices, and work closely with the Health Services Coordinator.
