We are seeking a skilled Clinical Nursing Specialist to join our team at Ely Manor Careers.
About the Role:
- This position is responsible for completing the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated timeframes.
- The successful candidate will assess residents through physical assessment, interview, and chart review, discussing their care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- They will also review information from hospitals, consultants, and outside agencies, using this information in the completion of assessments and care planning.
Responsibilities:
- Complete MDS, CAA's, and care plans within regulated timeframes.
- Assess resident needs through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers.
- Review information from hospitals, consultants, and outside agencies.
- Coordinate significant changes in resident care.
- Maintain PPS boards for monitoring Medicare days and RUGs utilization.
Requirements:
- Registered Nurse or Licensed Practical Nurse.
- AANC certification preferred.
- Knowledge of Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse.
