Responsibilities
The successful candidate will be responsible for conducting assessments and care plan coordination for residents assigned to their care. Key responsibilities include:
- Completing MDS, CAA's, and care plans within regulated time frames.
- Assessing resident needs through physical assessment, interview, and chart review.
- Discussing resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Reviewing information from hospital consults and outside agencies and utilizing such information in assessment and care planning.
- Coordinating and identifying significant change in MDS.'
Qualifications
To be successful in this role, candidates must possess:
- A Registered Nurse or Licensed Practical Nurse degree.
- AANC certification is a plus, with RAC-CT experience preferred.
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
- Experience as an MDS Nurse.
