Join Our Team as an MDS Nurse
We are seeking a skilled and experienced Registered Nurse or Licensed Practical Nurse to join our team as an MDS Nurse. In this role, you will have the opportunity to remain clinically involved in patient care while working independently with autonomy.
About the Role
As an MDS Nurse at Ciena Healthcare, your primary responsibilities will include:
- Completing the MDS, CAA's, and care plans within regulated time frames
- Conducting assessments and care plan coordination for assigned residents
- Discussing resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff
- Maintaining up-to-date knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS)
Responsibilities
Some key responsibilities of the MDS nurse include:
- Assessing residents through physical assessment, interview, and chart review
- Coordinating and initiating significant changes to MDS assessments as needed
- Preparing for and conducting PPS meetings, maintaining MDS assessments according to Medicare schedules, and monitoring Medicare days and RUGs utilization
- Remaining current with American Association of Nursing Assessment Coordinators (AANAC) requirements
Qualifications
To be successful in this role, you will need:
- Registration as a Registered Nurse or Licensed Practical Nurse
- AANC certification is a plus, with RAC-CT preferred
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS), with experience as an MDS Nurse strongly preferred
