Responsibilities and Requirements
- Leadership: Supervise and mentor a team of social workers to achieve excellent outcomes.
- Resident Care: Collaborate with facility staff to identify and address resident needs and concerns.
- Discharge Planning: Develop and implement effective discharge plans to ensure safe transitions for residents.
- Education and Experience: Bachelor's Degree in Social Work; Master's Degree preferred. One year of supervised social work experience in a healthcare setting. Current Social Work license from the state of MI.
