Job Summary
The Director of Social Services will oversee the social work team and ensure the well-being and satisfaction of our residents. This includes completing admissions, social services evaluations, and providing for social services or referrals.
- Responsibilities:
- Supervise social workers and guide facility staff in matters related to residency advocacy, protection, and promotion of residents' rights.
- Assist residents with financial and legal matters, such as applying for pensions and referrals to lawyers.
- Provide discharge planning services, including helping to place residents on waiting lists for community congregate living.
- Ensure accurate and timely documentation in MDS care plans.
