Job Description: Director of Social Services
- Provide expert social services to help our residents achieve their full potential.
Responsibilities:
- Oversee the Social Workers and guide facility staff in matters of residency advocacy, protection, and promotion of resident's rights.
Key Responsibilities Include:
- Developing and implementing social services strategies to enhance resident well-being.
- Coordinating admissions, social services evaluations, and referrals to optimize resident outcomes.
- Providing guidance and support to guests with financial and legal matters, ensuring they receive necessary resources and assistance.
- Collaborating with external agencies to develop comprehensive discharge plans that meet the unique needs of each resident.
Qualifications:
- Education and/or Experience:
- Minimum of Bachelor's Degree in Social Work. Master's Degree preferred.
- One year supervised social work experience working directly with individuals in a health care setting.
- Certificates, Licenses, Registrations:
- Current Social Work license from the state of MI.
