Responsibilities
- Supervise and guide Social Workers in matters of residency advocacy, protection, and promotion of residents' rights.
- Complete admissions, social services evaluations, provide social services or referrals, and maintain communication with residents and families.
- Assist guests with financial and legal matters, such as applying for pensions or referrals to lawyers.
- Provide discharge planning services, including helping guests find community congregate living arrangements or home care services.
- Arrange counseling services and psychiatric medication management as needed.
- Ensure accurate and timely documentation in MDS care plans.
- Supervise, hire, train, and direct Social Workers, using independent judgment and discretion on behalf of the organization.
Qualifications
- Minimum Bachelor's Degree in Social Work; Master's Degree preferred.
- Strong communication and teamwork abilities.
