Job Description: Director of Social Services
- Provide medically-related social services to help guests attain and/or maintain their physical, mental and psychosocial well-being.
Responsibilities:
- Supervise the Social Workers and guide facility staff in matters of residency advocacy, protection and promotion of resident's rights.
Key Responsibilities Include:
- Completing admissions, social services evaluations, providing for social services or referrals, and maintaining communication with resident and families.
- Assisting guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements.
- Providing discharge planning services, including helping to place a guest on a waiting list for community congregate living, arranging intake for home care services, assisting with transfer arrangements to other facilities, and working with outside agencies toward safe discharges.
- Providing or arranging needed counseling services and psychiatric medication management as indicated.
- Ensuring documentation is accurate and timely in MDS care plans.
Qualifications:
- Education and/or Experience:
- Minimum of Bachelor's Degree in Social Work. Master's Degree preferred.
- One year supervised social work experience working directly with individuals in a health care setting.
- Certificates, Licenses, Registrations:
- Current Social Work license from the state of MI.
