Absolute Elder Care is a geriatric focused Mobile Physician Group that provides Primary Care Services to patients in Assisted Living Facilities, Skilled Nursing Facilities, and Private Residences across Central and North Central Florida.
We are seeking a Nurse Practitioner to provide in-person visits to residents of assisted living facilities ( 17 visits per day) in the WINTER PARK / WINTER GARDEN / OVEIDO / CLERMONT area.
This is a part-time position. No weekends.
Core Responsibilities
- Work closely with our support staff to ensure patient needs are met timely to reduce hospitalization and improve quality of life
- Participate in weekly web meetings (via TEAMS) for physicians and advanced practice providers
- Participate in Medicare's Merit-based Incentive Payment System (MIPS)
- Complete documentation and billing in eClinical Works in accordance with Medicare and practice standards
- Conduct telehealth visits as needed
