Certification Details BLS ACLS Job Requirements BLS ACLS License / Certificate Verification Additional Details Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required. Positive MMR titers ONLY from within the last 15 years. Completed within 1 year of start updated annually. Will accept proof of full Hepatitis B series, or at least two vaccines from the series completed. If positive, CXR within 5 years and current Questionnaire. Positive Varicella titers ONLY from within the last 15 years. Proof of vaccination within past 10 years, declinations not accepted. If 2 step PPD not done within past year. Agency to provide completed I-9 or attestation that completed I-9 is on file. If not ran with background check, to be complete within 1 year of start. Live in Ohio for 5 years : Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check. Highest level of education verified. Must provide verification of employment within 5 years. Clinical Only : Skill specific required to be complete within 1 year prior to start. Competency Exams for the relevant department. Must score at least 80%. 80% passing score required, Completed within 1 year prior to start. Physical Exam completed within one year of initial start date. Proof of vaccine from Oct-March or exemptions must be submitted for approval. Non-expired photo ID provided. 2-Step TB test with negative reads within the last 6 months. Resume to include Education and Work History, 2 Professional References. Shoulders up, plain background, headshot. Ran within 6 months of Start. Search includes SSN Trace. Verified to be ran separately from background check. Supporting documentation that the individual was screened and cleared. Ran within 60 days upon initial start date to include the following drugs : Amphetamines, Barbiturates, Benzodiazepines. Safety and Compliance Training modules emailed to agency POC by UHS after acceptance.
