Job Summary:
As a Nurse Practitioner in Home Health, you will provide comprehensive, family-centered care to hospice patients in their homes, including developing and revising care plans, managing symptoms, and offering emotional and spiritual support. You will collaborate with physicians and interdisciplinary teams to monitor patient progress, ensure accurate diagnoses, and facilitate patient recertification through face-to-face encounters. Additional responsibilities include participating in policy development, quality assurance, and on-call rotations, while ensuring compliance with regulations and infection control practices. This role requires strong communication, clinical documentation, and assessment skills, along with 2 years of hospice experience and reliable transportation.
Qualifications:
- Registered Nurse Practitioner, licensed per the state requirements.
- 2 years of experience as a Nurse Practitioner
- 2 years of Hospice experience
- Good communication skills, assessment skills, and strong clinical documentation skills.
- Ability to read, write, and speak the English language.
- Reliable transportation. Valid and current auto liability insurance.
Essential Functions:
- Participate in clinical service activities, including policy development, staff orientation, and personnel qualifications.
- Gather and communicate clinical information to support diagnosis, prognosis, and recertification, including face-to-face encounter attestation.
- Develop, implement, and revise hospice care plans, involving patients and families in the process.
- Provide family-centered care across settings, including pain management, emotional support, and spiritual/cultural considerations.
- Deliver necessary nursing care and education to patients, families, and caregivers.
- Monitor patient progress and coordinate care with other disciplines, initiating referrals as needed.
- Maintain communication with care providers to ensure timely information sharing and decision-making.
- Notify physicians and IDG members of changes in the patients condition or environment.
- Conduct concurrent reviews to justify extended stays and document findings.
- Participate in education programs, hospice IDG meetings, and on-call rotations.
- Ensure compliance with regulations, infection control practices, and patient rights.
- Address quality of care issues and report to the Quality Assurance Committee.
- Provide positive customer relations with all stakeholders and adhere to hospice policies.
- Demonstrate mission and values in daily responsibilities and perform other related duties as assigned.
