What are the responsibilities and job description for the Transitional Care Coach - 2nd Shift position at EMBERHOPE CONNECTIONS, LLC?

To be a successful Transitional Care Coach you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Float between EmberHope programs to include Transportation, On-Call, and Connect 4 Success (day services), depending on the care and support for clients needed.
- Monitor the Connect 4 Success sites to maintain client safety in day services. Monitor and document client arrival and departure. Ensure safe, age-appropriate learning and activities for clients. Maintain client medication and attendance logs and safety sheets monthly for a report.
- Organize and maintain activities, supplies, and video equipment. Make sure documentation forms are up to date and placed in the sign-in and out areas. Responsible for organization of supplies and cleanliness in the Connect 4 Success sites.
- Provide on-call crisis response during designated shifts, on-call may respond to crisis by phone but may also require in-person assistance including transporting clients.
- Safely transport children and youth and their personal belongings, medication, and appropriate documents to their placements and appointments.??
- Participate and complete Trauma Based Relational Intervention (TBRI), trauma informed training, and Safe Crisis Management (SCM) to stay current on working with high needs children or behaviors of concern.
- Coach and mentor children and youth in a success focused manner. Provide positive role modeling and establish a professional rapport with clients supervised.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.

- 25% Coaching & Mentoring Clients in Connect 4 Success sites.
- 25% Transportation of Clients
- 25% On-Call Support
- 15% On-going Training
- 10% Paperwork and Critical Incident Reporting
To qualify for this job, you need to have:
- Degree Requirements - H.S. diploma or GED required.? Bachelor’s degree preferred.
- Licensure - Good driving record, valid Kansas driver’s license and insurability through company’s carrier.?
- Work Experience – Child welfare, juvenile justice, and/or mental health serving adolescents or young adults' preference is given. Experience working directly with youth with challenging behaviors, required.
- 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
- Maintain company-required training.
