What are the responsibilities and job description for the Aftercare Specialist Non-Licensed position at EMBERHOPE, INC?

To be a successful Aftercare Specialist you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide weekly phone contacts and monthly in-person contacts with all family members. Coordinate contacts and meetings that support the family’s schedules. Assess the family’s needs and follow up on any accessibility to services issues.
- Staff and collaborate with case managers to process the case and create a smooth transition of case supervision. Initiate and facilitate case transfer meetings with the Department of Children and Families (DCF). Request consultations with DCF as needed.
- Focus on engagement efforts with the family throughout the duration of the aftercare period.
- Work with the family and case team to create and monitor an Aftercare Plan.
- Search for and provide means for the family through community resources. Assist Another Planned Permanency Living Arrangement (APPLA) youth with accessing concrete goods and services to assist with maintaining stability, safety, permanency, and wellbeing.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
- Back up on-call to the on-call department.
