What are the responsibilities and job description for the Crossover Permanency Specialist, Non-Licensed position at EMBERHOPE, INC?

To be a successful Crossover Permanency Specialist you must be able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Work with youth who move between the juvenile justice and child welfare systems utilizing the Crossover Youth Practice Model (CYPM) to address the unique needs of the youth.
- Develop partnering relationships with parents and youth, with the goal to work toward reunification. To help achieve reunification as soon as safely possible, permanency and specialized case management, engage the family in a meaningful relationship and to utilize assessment and case planning to guide services.
- Support youth and families by collaborating with a variety of specialized service areas within the program. Assist them in connecting with specific community resources and specialized services to benefit the family.
- Ensure documentation is complete and/or upload timely according to company policy and procedures.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
- Back up on-call to the on-call department.
