To be a successful Hope Unified for Belonging Support Worker (HUB Support Worker) you must become able to do the following always:


The Hope Unified for Belonging Support Worker (HUB Support Worker) provides direct assistance and support to youth in foster care who are experiencing placement instability, working closely with the HUB Specialists to implement individualized plans of care and facilitate the youth's successful transition to stable placements. The HUB Support Worker assists with daily living activities, transports youth to appointments and activities, provides emotional support and guidance, and collaborates with the HUB team to ensure the youth's needs are met effectively.


  • Assist HUB Specialists with the implementation of individualized plans of care for youth in the HUB program.
  • Provide direct support to youth in daily living activities, such as hygiene, meal preparation, and organization.
  • Transport youth to appointments, court hearings, school activities, and other community-based activities.
  • Offer emotional support and guidance to youth, utilizing trauma-informed and strengths-based approaches.
  • Assist with crisis intervention and de-escalation techniques as needed, under the guidance of the HUB Specialist.
  • Collaborate with the HUB team to ensure the youth's basic needs are met and their individualized needs are addressed.
  • Maintain accurate and timely documentation of services provided and observations of the youth's progress.
  • Participate in regular supervision and team meetings to discuss the youth's needs and progress.
  • Adhere to all agency policies and procedures, ensuring the safety and well-being of the youth served.
  • Assist with other program-related tasks as assigned by the HUB Supervisor.
  • Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery. Review and approve required documents and reports as needed.
  • Partner with the Data Services Department on the Electronic Health Record (EHR) processes and workflows.
  • Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
  • Participate in on-call rotation.

To qualify for this job, you need to have:

  • Education: High school diploma or equivalent required. Bachelor's degree in social work or a related field preferred.
  • Experience: One (1) to two (2) years of experience working with youth in foster care or a related field preferred.
  • Training: Must have completed or be willing to complete Trust-Based Relational Intervention (TBRI) and Safe Crisis Management (SCM) training.
  • 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
  • Maintain company-required training.
  • Ability to work a flexible schedule, including some evenings and weekends.
  • The employee is subject to outside environmental conditions occasionally, and inside conditions frequently, with protection from weather conditions but not necessarily from temperature change. (Physical working requirements).
  • Meet the following shift structure requirements:
  • To ensure comprehensive coverage and support for the youth in the HUB program, the following shift structure and on-call rotation will be implemented:
  • Shift Structure: The HUB team will consist of two shifts:
  • First Shift: 6:00 AM - 2:00 PM (2 HUB Specialists and 1 Support Worker)
  • Second Shift: 2:00 PM - 10:00 PM (2 HUB Specialists and 1 Support Worker)
  • Crossover: There will be a one-hour crossover between shifts (2:00 PM - 3:00 PM) to ensure shared information and staffing time for continuity of care.
  • Weekend On-Call: The HUB team, including the six staff members and supervisor, will have an on-call rotation on the weekends to respond to the needs of the 20 youth in the program.
