What are the responsibilities and job description for the Relative Assessor position at EMBERHOPE, INC?
To be a successful Relative Assessor you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide services that result in timely and safe achievement of company and state outcomes relevant to the position.
- Exercise discretion and judgement in the best interest of children served. Provide knowledge, skills, and tools to children and families to successfully function within the expectations and procedures outlined in the Department of Children & Families (DCF) policy and procedure manual as well company policy and procedures. Teach skills such as emotion regulation, having healthy relationships, conflict resolution, and healthy communication within families.
- Conduct family visits and monthly supervision of the home.
- Completes assessments and makes recommendations for placement with relatives.
- Completes KBI, CANIS, Adult Abuse Registry background checks and fingerprints for prospective relative families.
- Recommend services needed by the child and family to maintain placement in the relative home.
- Completes safety plans, as needed, with relative families.
- Completes and maintains written documentation in the family file accurately and within established timeframes.
- Participates in individual and team meetings with a focus on sharing pertinent information and making improvements in services.
- Provide crisis intervention as needed.
- Supports kinship workers and relative families to maintain placements and help children work toward permanency.
- Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
- Ensures clients’ rights are protected. Makes sure all clients have received their rights and maintains a signed copy in each file.
- Is knowledgeable of and follows all safety procedures.
- Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
