What are the responsibilities and job description for the Independent Living Support Worker position at EmberHope?
To be a successful Permanency Support Worker you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
- Provide direct supervision and support of supervisees utilizing the agency model for supervision, Supervision for Success.
- Coordinate and supervise client visits with families. Assist with completion of assessment packets and monthly contact. Provide parenting and life skills training and development. Implement tasks identified on case plans to help youth reach goals. Attend case plan and Benefit review meetings as needed. Assist youth with identifying, locating, and accessing basic resources and community support systems.
- Document 100% of direct services provided to the youth 48 hours of the occurrence. Submit required monthly reports to supervisor. Complete visitation record requests.
- Transport clients as needed. Travel in state to assure client contact as needed.
- Collaborate with Permanency Specialist, Permanency Services Team, internal, and external stakeholders, demonstrating problem-solving/facilitation skills. Participate in individual and Permanency Services Unit/Dyad supervision once a month or as needed with the supervisor.
- Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
