The Intake and Assessment Clinician is responsible for facilitating the referral process and participating in the review of client acceptance and denials for our competency restoration residential programs. The Intake and Assessment Clinician will perform assessments and screens for appropriateness. This is a fulltime staff position within a 24/7 program and may require night and weekend coverage for the safety and well-being of all clients residing in the facility.
Essential Duties And Responsibilities
Education/Experiance
Master’s degree in a social service field. Unlicensed and licensed individuals considered.
At least one year’s experience involved in mental health assessments or placements, residential environments or a related field.

Essential Duties And Responsibilities
- Conducts thorough assessments to understand a client's needs, strengths, and challenges.
- Collects relevant information through interviews, observations, and review of records.
- Identifies the client's specific needs and determine the most appropriate services and treatment options.
- Develops individualized service plans based on the assessment findings.
- Connects clients with appropriate resources and services, ensuring smooth transitions and ongoing support.
- Receives and reviews referrals from external agencies to determine the appropriateness of placement, while ensuring accurate and timely processing of all referrals.
- Coordinates all aspects of referral steps, program interview, intake assessments and program entry, updates and maintains ongoing communication with referral sources and completes data entry through disposition.
- Analyzes and tracks referral data and develops reports to determine trends, issues, etc.
- Participates in case conferences and team meetings to ensure effective integrated service delivery related to referral and intake process.
- Maintains accurate and timely documentation of assessments, service plans, and client interactions in accordance with contract and compliance requirements.
- Assembles referral information for internal and external parties.
- Works collaboratively with other healthcare professionals, community partners, and outside agencies to ensure comprehensive care.
- Coordinates and facilitates the transfer of new clients to facility.
- Provides immediate support and intervention during crisis situations.
- Displays decision-making authority over referrals that are presented for acceptance.
- Conducts groups and individual sessions related to the initial intake of accepted referrals.
- Understands and promotes recovery-based treatment and trauma-informed practices.

- Develops and maintains good working relationships with all departments, the management team, regulatory agencies, and community partners.
