YOU have an exciting opportunity to join our team and work at our exceptional Skilled Nursing Facility! We are currently expanding our staff and are looking for a compassionate and enthusiastic Social Worker assistant to work with our already wonderful staff.
Knowledge of : Care plans, Admissions data, BIMS, NOMNOCs, PHQ-9, PCC, Medicaid / insurance basics, prefered.
Must be a self starter who can complete tasks without assistance once trained.
Must have general knowledge on Microsoft office programs. (Teams, Excel, Word, Outlook etc)
Can pass a Background check and APS / CPS report
