YOU have an exciting opportunity to join our team and work at our exceptional Skilled Nursing Facility! We are currently expanding our staff and are looking for a compassionate and enthusiastic Social Worker assistant to work with our already wonderful staff.
- Will assist the Social Services Director in the completion of assessments and documentation pertaining to the psychosocial health and wellbeing of our residents.
- Will assist the Social Services Director in managing the Grievance Resolution process for residents and family members.
- Will meet with residents on a regular basis to ensure satisfaction with the service they are receiving from our staff and providers.
- Experience in customer service, Social Services, Activities, or as a caregiver in a skilled nursing or acute care environment preffered.
Knowledge of : Care plans, Admissions data, BIMS, NOMNOCs, PHQ-9, PCC, Medicaid/insurance basics, prefered.
Must be a self starter who can complete tasks without assistance once trained.
Must have general knowledge on Microsoft office programs. (Teams, Excel, Word, Outlook etc)
Can pass a Background check and APS/CPS report
