What You’ll Be Doing as a Nurse Practitioner – In-Home Primary Care (Field-Based Role)
- Conduct in-home primary care visits, including comprehensive health assessments and chronic disease management.
- Develop and implement individualized care plans for adult patients with acute and chronic health conditions.
- Educate patients and families on health management strategies, medication adherence, and preventive care.
- Collaborate with a multidisciplinary team to ensure continuity of care and optimal patient outcomes.
- Document patient interactions using an EMR system (Athena preferred) to maintain accurate records.
What You’ll Bring as a Nurse Practitioner – In-Home Primary Care (Field-Based Role)
- Certified Nurse Practitioner (NP) or Physician Assistant (PA) from an accredited program.
- Minimum of 2 years of clinical experience in primary care or home health, with a demonstrated ability to work independently.
- Active, unrestricted state license (or eligibility) and a valid driver’s license.
- Current DEA registration (or eligibility) and Medicare/Medicaid enrollment (or eligibility).
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
- Proficiency with EMR systems and familiarity with Microsoft Office.
