We are seeking enthusiastic Intake Specialist(s) to join our team! Areas needed:
Greensburg, PA
Qualifications:
- Bachelor's degree or equivalent homecare experience.
- Minimum of 2 years of experience in a homecare or other healthcare setting, with a focus on client care.
- Strong understanding of non-medical homecare services.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and staff.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
- Proficiency in using electronic medical records (EMR) and other client management software.
- Flexibility to work occasional evenings or weekends as needed to meet client needs and conduct supervisory visits.
- The role may involve lifting, bending, and other physical activities related to client care and home visits.
What you'll do for our clients:
Client Care Management:
Conduct initial intakes and conduct home assessment with the client and their families.
Quality Assurance and Supervisory Visits:
- Conduct quality assurance visits to clients' homes to assess the quality of care being provided and ensure compliance with agency standards.
- Perform supervisory visits to monitor caregiver performance.
- Document and report findings from quality assurance and supervisory visits.
Communication and Collaboration:
Participate in staff meetings and training sessions to stay informed about agency policies, procedures, and best practices.
