What are the responsibilities and job description for the Community Outreach Social Worker position at Emergency Aid Association DBA Suffield Community Aid?
Suffield Community Aid (SCA) is a unique and private, non-profit 501(c)(3) social services agency that has served Suffield, CT residents as a “safety net” during times of need for over 115 years. Our mission is to provide supportive health and social services that positively affect the well-being of Suffield residents.
We have an excellent opportunity for a Community Outreach Social Worker to join our team. Reporting to our Executive Director, you will help to provide social work and outreach services to older adults, adult residents with disabilities, as well as individuals and families in need of financial, social, physical and/or emotional support.
Primary Responsibilities:
- Assess client needs in collaboration with clients, family, and caregivers, conducting both in-office and home visits.
- Provide case management, needs assessments, information, and referral services, continually monitoring and modifying services.
- Advocate for clients, assisting in the application for services and benefits.
- Make appropriate referrals, ensuring confidentiality and autonomy preservation.
- Conduct outreach to serve less visible residents, coordinate support groups, and foster community linkages.
- Utilize various media for community outreach and maintain electronic databases with confidential data.
- Assist in coordinating and delivering local programs, including screening for various services.
- Serve as Suffield’s “Municipal Agent for the Elderly” and participate in short/long-term planning.
- Coordinate the Friendly Caller and Visitor Program, including volunteer recruitment, training, and supervision.
- Maintains updated “At-Risk List” identifying vulnerable residents in the Suffield community.
- Potentially supervises BSW or MSW student interns for their field placement learning.
- Stay informed and join participate in professional organizations to stay current with relevant policies, programs, and legislative changes affecting the served population.
- Other duties developed over time and as assigned.
Qualifications:
- Master of Social Work (MSW) degree and Connecticut Social Work license required.
- 1 years of experience providing social services support to older adults, adults with disabilities, and/or financially at-risk individuals and families in a similar setting.
- Experience using Microsoft Office, Excel, and PowerPoint. Familiarity with Charity Tracker, a plus.
- 1 years of experience in crisis intervention.
- Willing to move medium to heavy (20-40 pound) boxes and medical equipment, travel in-state and work occasional evening and weekend hours.
- Valid driver’s license.
- Must be willing to submit to and pass a background check and drug screening.
Salary Range & Benefits:
- $70,000 - $85,000 annually; salary commensurate with experience.
