Eminence Home Care is actively seeking a dedicated and compassionate Patient Coordinator and Assessment Specialist (PRN) to join our dynamic team. In this essential role, you will be responsible for conducting patient assessments, coordinating care plans, and ensuring that our clients receive the highest level of service. Your expertise will play a vital role in supporting our clients and their families as they navigate their healthcare journey.

You will:
  • Conduct comprehensive assessments for new patients to determine their healthcare needs and preferences.
  • Collaborate with patients, families, and caregivers to develop personalized care plans that address individual requirements.
  • Coordinate with various healthcare providers to ensure seamless communication and collaboration in patient care.
  • Monitor patient progress, adjust care plans as needed, and ensure adherence to quality standards.
  • Provide education and resources to patients and families regarding care options and services available.
  • Document all assessment findings, care plans, and patient interactions accurately and in a timely manner.
  • Participate in team meetings and contribute to the continuous improvement of care services.

You will have:
  • Current RN or LPN license preferred but not required.
  • Previous experience in a healthcare setting, particularly in patient assessment or coordination roles.
  • Strong interpersonal and communication skills with the ability to build rapport with patients and families.
  • Ability to work independently and manage multiple priorities effectively.
  • Highly organized with strong attention to detail.
  • Proficiency in EMR systems, preferably HHAeXchange, and Microsoft Office.
