Job Summary
We are seeking a compassionate and dedicated Hospice Chaplain/Bereavement Coordinator to join our team. This role involves providing spiritual support to patients and families, as well as managing the bereavement services for those who have lost loved ones. As a Hospice Chaplain, you will offer emotional and spiritual care to individuals facing end-of-life issues, regardless of their religious or spiritual background. As a Bereavement Coordinator, you will oversee the grief and loss services, helping families through their grieving process and providing ongoing support.
Duties
- Provide spiritual care and counseling to patients and families based on individual preferences and needs, offering comfort and guidance during end-of-life care.
- Conduct spiritual assessments and create personalized care plans for patients and families.
- Serve as a liaison between the hospice team and the family, offering support and advice on spiritual matters and end-of-life concerns.
- Collaborate with the multidisciplinary team to provide holistic care, including emotional, physical, and spiritual support.
- Develop and manage bereavement programs to support families before and after the loss of a loved one.
- Provide grief counseling to individuals and groups, ensuring families have the tools and support they need to cope with their loss.
- Monitor and document the progress of families throughout the bereavement process and adjust services as needed.
- Provide ongoing support and outreach to families through phone calls, letters, and home visits as required.
- Ensure compliance with regulatory requirements regarding bereavement care and documentation.
Experience
- Bachelor's degree in Theology, Divinity, or a related field (Master’s preferred).
- Certification as a Chaplain (e.g., Board-Certified Chaplain or equivalent).
- Experience working as a chaplain, preferably in a healthcare or hospice setting. (not required)
- Strong interpersonal and communication skills, with the ability to work well with diverse populations.
- Ability to provide spiritual care and emotional support in a compassionate and non-judgmental manner.
- Familiarity with hospice care models and the special needs of individuals facing end-of-life issues. (preferred)
- Ability to work collaboratively as part of an interdisciplinary team.
